What Does the VA Look for When Rating Migraine Headaches?

The VA rates migraine headaches as a neurological condition. The rating is based almost entirely on how often and how severely your migraines affect your ability to function. The VA uses the term "prostrating attacks" to describe the migraines that count most toward your rating.

A prostrating attack is one that forces you to stop what you are doing and lie down or rest. It does not have to put you in the hospital. It just has to be severe enough that you cannot continue normal activity while it is happening.

What Are the VA Rating Levels for Migraines?

The VA assigns disability ratings for migraines at four levels:

  • 0 percent: Less frequent attacks that do not significantly affect daily life

  • 10 percent: Prostrating attacks happening once every two months or less frequently

  • 30 percent: Prostrating attacks occurring more than once per month

  • 50 percent: Very frequent, completely prostrating and prolonged attacks that produce "severe economic inadaptability"

The 50 percent rating is the maximum rating for migraines. Reaching it requires showing that your migraines are so frequent and severe that they make it extremely difficult or impossible to keep a regular job. 

How Do You Prove a Military Service Connection for Migraines?

Before the VA will rate your migraines at all, you have to show that they are connected to your military service. This is called service connection and it is the foundation of your entire claim.

Building a strong service connection argument generally requires:

  • A current diagnosis of migraine disorder from a medical provider

  • Evidence of an in-service event, injury or illness that could have caused or contributed to your migraines

  • A letter from a medical professional directly linking your current condition to that in-service event, called a "medical nexus opinion"

The nexus opinion is often the most difficult piece to obtain. This is a statement from a doctor explaining that your migraines are at least as likely as not caused or worsened by your military service. Without it, even a well-documented claim can be denied.

Common service-related causes of migraines include traumatic brain injuries, exposure to toxic substances, combat-related stress, and neck or head injuries. If you were exposed to burn pits or other hazardous materials, the PACT Act may also give you other options for your claim.

What Are the Most Common Reasons VA Migraine Claims Get Denied?

The VA denies migraine claims for several common reasons. Understanding them ahead of time can help you avoid the same pitfalls:

  • Not enough documentation of how often prostrating attacks occur

  • Lack of a medical nexus connecting migraines to service

  • Inconsistent medical records that underreport migraine frequency or severity

  • Missing buddy statements or personal statements describing the real-world impact of attacks
